(1) The Director shall issue a SPL in the appropriate prescribed form, as by the Director, if the applicant complies with the requirements referred to in regulation 61.02.2.
(2) Upon receipt of the SPL, the holder must immediately affix his or her signature thereon in ink in the space provided for such purpose.
